The case manager for the EMPOWER clinic will provide comprehensive case management services for survivors of commercial sexual exploitation (CSE) and sex trafficking. They will utilize a trauma informed approach to promote clients right to self-determination and provide services in a manner that emphasizes safety, respect and dignity. They will provide strength-based guidance and work to resolve barriers to achieving self-sufficiency.

They will provide strength-based guidance and work to resolve barriers to achieving self-sufficiency.</p> <p> </p> <img src="https://analytics.click2apply.net/v/aQOXEEcoGpX5T1Qph8mA5"> <br/><br/><p>Primary Job Responsibilities:</p> <p>List and explain the core / primary responsibilities of the job.</p> <p>Assesses immediate needs of clients and makes appropriate referrals, including shelter/emergency transfer, and medical treatment.</p> <p>Assists clients in obtaining public benefits for which they are eligible (e.g. Medicaid, food stamps, housing and childcare subsidies).</p> <p>Assists clients in navigating school system and educational advocacy</p> <p>Assists clients in creating stability for themselves and their family through appropriate referrals for counseling, financial counseling, ESOL classes, job readiness programs and higher education.</p> <p>Refers clients to other appropriate legal and social services both within and outside of Sanctuary.</p> <p>Provides substance use recovery support and referrals.</p> <p>Prepares requests for educational and financial assistance grants on behalf of clients.</p> <p>Provide limited on-site childcare at EMPOWER center during clients appointment *</p> <p>Advocates on behalf of clients.</p> <p>Maintains accurate case records, documents all services provided within Sanctuarys Service Tracking System and prepares statistics for monthly reports.</p> <p>Performs administrative support.</p> <p>Assist clients in reducing barriers to accessing care.</p> <p>Assist clients to identify goals, strengths, and personal interests.</p> <p>Work with clients to reduce risk in their lives.</p> <p>Perform any other department or agency related duties or special projects as directed.</p> <p> </p> <p>Education/Training/Experience:</p> <p>What education, certification, formal training, and years of experience are necessary to successfully perform the job?</p> <p>Bachelors Degree required</p> <p>At least one year experience in the social services field, preferably in CSE/trafficking, street outreach, intensive case management, knowledge of substance use, foster care, homeless population, pregnant and parenting populations, low-income populations, people with high medical needs, navigating law enforcement and the criminal justice system, working with individuals involved with gangs and knowledge of multi-generational poverty</p> <p>Comfortable working with all ages</p> <p>Organizational Relationships:</p> <p>What organizational relationships are necessary to successfully perform the job?</p> <p> </p> <p>Works closely with EMPOWER peer care navigators</p> <p>Works in collaboration with EMPOWER clinic team</p> <p>Collaborates as needed with Sanctuary for Families legal, clinical and economic empowerment departments as needed</p> <p>Maintains strong linkages with partner agencies</p> <p>Develops strong partnerships with trauma-informed community based organizations</p> <p>Knowledge and Skill Requirements:</p> <p>What knowledge and skills are most important to successfully perform the job?</p> <p>Fluency in both English and Mandarin or Spanish required</p> <p>Strong verbal/written communication and interpersonal skills</p> <p>Ability to work independently, multitask and prioritize</p> <p>Familiarity to Microsoft Office, including Outlook, Work, and Excel</p> <p>Management Responsibilities:</p> <p>Please describe any management responsibilities, including the management of staff, a particular function, and/or budget.
