General Purpose

The Family Life Coach provides voluntary comprehensive, coordinated case management services to families experiencing instability. This position is within the Colorado Community Response (CCR) program, with the purpose of engaging families and connecting them to community services to prevent deeper penetration into the child welfare system. The Life Coach will offer education and support services to parents, using strength-based practices. Responsibilities include conducting initial and follow-up assessments; working cooperatively with other agencies to link families to financial services and community resources that meet the parents' expressed needs; and providing direct services and accurate data entry to track and record contacts. Flexible schedule required as this position will work occasional evenings and weekends as needed with some sessions provided in the client's home.

Essential Duties and Responsibilities

  • Call referred families providing program outreach for enrollment, offering case management and program services. Follow CCR protocols for required follow-up for prospective program participants.
  • Engage and interview families for program enrollment and provide needs assessment using the CFSA2. Offer case management services, and follow-up contacts to assess goal attainment and need for further referrals. Manage a caseload of 5-10 families at a time.
  • Conduct home visits or other location convenient for family (weekly, semi-monthly, monthly) as determined by family to provide needs assessment, establish client rapport, set family-centered and strengths-based service plan, and provide direct services and follow-up to assigned families.
  • Follow-up contacts to assess goal attainment and need for further referrals and providing resources such as financial planning, parenting skills and communication skills.
  • Facilitate and coordinate assistance flex funds of last resort based upon defined family goals to create greater financial and family stability.
  • Arrange community referrals for services to maintain a safe environment and enhance child and family wellbeing. Referrals include meeting basic, safety, social, esteem, and cognitive needs of individuals within the family and/or the family as a whole. Intensive follow-up required for any referrals provided.
  • Develop effective case plans for families regarding parenting skills, family problems, economic stressors, parent/child relationships and community connectedness. Work toward keeping children safe in their home and the community. Participate in ongoing trainings and be responsible for following Quality Standards for Family Strengthening & Support and Trauma-Informed Care.
  • Participate in state meetings and trainings, with the approval of supervisor, in order to implement new and ongoing rules and regulations.
  • Participate with regional CCR hubs for programmatic information sharing and program support.
  • Support and maintain relationships with county DHS partners.
  • Provide advocacy to entities involved with family, based on family needs, including but not limited to welfare and public benefit agencies, landlords, and educational entities.
  • Report to supervisor including information on number of referrals received, home visits completed, allocation of flex funds as part of report sent to funders and research team.
  • Maintain accurate reports and databases of client demographics and services provided for clients. Use appropriate forms to collect demographic and program information, for appropriate documentation of case plans, recommendations, contacts, assessments, family engagement activities, etc. Assist with tracking expenditures/revenues to comply with program budget.

Part Time - 20 hrs/wk

FLSA Classification


  • Bachelor's degree in Human Services, Social Work or related field, or a combination of experience and/or education equal to two years.
  • Two years of full-time experience working with direct client/family interaction.
  • Bilingual English/Spanish preferred.
  • Fingerprint and background clearance required.
  • Transportation and valid driver's license required.
  • Demonstrated ability to apply culturally appropriate skills in interactions with clients, co-workers, volunteers, partner agencies, and other community members.
  • Excellent communication (verbal and written), problem solving, listening, and interpersonal skills, with the ability to form and maintain positive work relationships.
  • Occasional evenings and weekends are required to accommodate meetings and special events.
