Job Summary: The JET Express/Joshua’s Path care manager position is a 22 hour a week position supporting members in the JET Express volunteer driver program and Joshua’s Path, JFS’ program to support caregivers to an adult with a developmental and physical disabilities. Case management will be provided by consulting and coordinating with individuals and/or families in maintaining and/or improving their independence, level of functioning and quality of life by providing for and/or arranging for an array of social services. This is a performance-based position.
Perform bio-psychosocial assessments including the areas of physical functioning, psychological/emotional functioning, mental status, activities of daily living and instrumental activities of daily living, social functioning, nutrition and environment. These assessments help determine the client’s needs and are performed in client’s home setting.
Develop care plans based on assessment to meet client’s needs and to establish a working relationship.
Refer for and coordinate needed services internally and externally. Advocate for the needs of the client. Consult and collaborate with other professionals and service providers to ensure comprehensive care.
Counsel, educate and support clients’ families and significant others to maximize their well-being.
Monitor the ongoing needs of clients through telephone and face-to-face interaction in order to evaluate the treatment process.
Document all activity and maintain electronic case records to meet quality standards.
Maintain caseload size as required by the agency to meet goals.
Assist with incoming care management inquiries and follow up calls.
Respond to all communications in a timely manner for care continuity.
Enter client information and case notes into an electronic data base.
Prepare/enter statistics for outcome measurement.
Interface with other professionals and agencies to coordinate care.
Attend meetings including staff, department, in-service, CEU programs to enhance communication.
Perform administrative functions such as copying, typing, mailing, filing to meet organizational needs.
Notify management for assistance when physical demands or environmental conditions might represent an increased risk for injury.
Graduate degree from an accredited school of social work or other behavioral mental health program. A degree from an accredited school of nursing is also acceptable. A certification in care management is a benefit but not a requirement.
Case management experience with older adults, geriatrics and special populations and their family members is preferred. Experience with Medicaid-Waiver benefit coordination and residential placement is a plus.
A valid Florida driver’s license is required.
Competent in Microsoft Outlook, Excel and the ability to learn an electronic health records database.
Other skills important for this position include problem solving, computer knowledge, analytical ability, communication and interpersonal skills, patience, flexibility and ability to function independently. An understanding of Jewish values and traditions is a plus, but not required.
