The School of Medicine, Departmentof Pediatrics is seeking a Community Outreach Worker Ifor the Department of Immunology. Under direct supervision, this position providesoutreach and adherence-to-care services for youth and young adults with theSTAR TRACK program. STAR TRACK (Special Teens At Risk Together Reaching Access,Care, and Knowledge) provides HIV treatment and care services to youth andyoung adults living with HIV or at-risk for HIV. Outreach services primarilywill take place at community venues, such as high schools, shelters,alternative schools, and local colleges and universities. This position willfocus primarily on working with youth and young adults living with HIV toidentify barriers to care and treatment and work to address those barriers.This position will also work with youth and young adults at risk for HIV andwho are enrolled STAR TRACK's PrEP program.

Recruits a client base within thecommunity by identifying, locating, and screening individuals who may beappropriate for the STAR TRACK program.
Provides general information togroups and individuals on STAR TRACK's program objective and services;eligibility requirements and benefits; confidentiality of information.Distributes informational materials and literature.
Serves as a liaison between theclient and community resources including department staff, City, State, andFederal social services agencies.
Initiates client appointments withhealth care providers, reminds them of pending appointments, and contacts themto inquire into reasons for missed appointments. Escorts clients to variousappointments to ensure compliance and provide support.
Assists with client retention byfollowing up on all contacts for continued progress assessment and locatingthose clients who have moved or lost contact with the STAR TRACK program.
Assists with planning, organizing,and implementing community special events such as health fairs, workshops, etc.Updates information in database from various data forms collected.
Education: High-school diploma or GED.
Ability to communicate effectively orally and in writing. Ability to establishand maintain effective working relationships. Ability to operate computers andother office equipment. Ability to write reports and maintain records. Abilityto work in extreme weather conditions. Ability to perform extensive standingand walking. Ability to handle sensitive and confidential matters withdiscretion and tact. Ability to interact with the public, health careproviders, community organizations, and social service agencies. Ability tomaintain poise and courtesy under pressure.
Other: Except for qualificationsestablished by law, additional related experience and formal education in whichone has gained the knowledge, skills, and abilities required for fullperformance of the work of the job class may be substituted for the educationor experience requirement on a year-for-year basis with 30 college creditsbeing equivalent to one year of experience.
Conditions of Employment: Employeesin this job class may be required to use their personal automobile whileconducting official business and possess a valid Maryland Non-commercial ClassC or Commercial Class B Driver's License (CDL). Candidates selected foremployment may be subjected to medical inquiries and/or medical examination todetermine ability to perform the job. Employees in this job class may berequired to successfully pass a police background check.
Based on the duties of this position, selectedcandidates are eligible to participate in the Bargaining Unit (Union).
SalaryRange: $27,600– $32,000
