This Patient Navigator will be responsible for:
Working alongside other care team members, including pediatricians, nurses, and social workers to provide education and emotional support to families of young patients.
Helping guide families through the healthcare system and other community-based supportive care services.
Serving as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services within Boston Children's Hospital and in the community.
Facilitating patient appointments, including those made with labs, diagnostic areas and specialty physicians.
To qualify, you must have:
Two years of occupationallyspecific education or an Associate's degree in Human Services or related field with a minimum of one to 3 years of experience.
Excellent communication skills and the ability to provide empathy in difficult situations.
Fluency in Spanish, highly desirable.
