The Case Coordinator position (Sr. Community Health Program Representative) is the Peer Case Manager Role in the Early Psychosis Programs at the UC Davis Medical Center. Provides personal account of lived experience as a consumer of services within the social services sector (ie. Medi-cal recipient; CalFresh or WIC recipient; history of homelessness, foster care placement, receiving county mental health services, and/or Department of Rehabilitation participation) to consumers, families, educators, community members and treatment providers. Participates in weekly clinic team meetings, assist with community outreach and education efforts, provide education and case management support to consumers and their families as part of a clinical team, and assist consumers and their families in accessing community resources. Services may be provided in a variety of settings, including the clinic, the community, local schools, or the family home. Through contact with consumers and families within SacEDAPT, the Case Coordinator assists the treatment team in identifying individual, family, home or community conditions needing intervention and support the client and family in meeting those needs. Engages Spanish speaking clients and family members/support persons and assists in their ongoing care coordination as part of the culturally responsive model of care.REQUIRED:--College level education in psychology, social work, counseling or related field--Experience as a consumer of services within the social services sector (such as Medi-cal recipient; CalFresh or WIC recipient; history of homelessness, foster care placement, receiving county mental health services, and/or Department of Rehabilitation participation) with the ability to share experiences--Bilingual Spanish skills (oral & written)--Knowledge of community resources in Sacramento County.--Previous experience supporting individuals or families in accessing supports or services--Excellent oral and written communication skills to effectively work with physicians, consumers and families, clinical staff, research staff, and other health care professionals is required--Knowledge of and experience with various software programs (such as WordPerfect, Microsoft Word, Excel, Access), email systems and internet.--Organizational and time management skills to prioritize workload and meet deadlines, develop and carry out project assignments in an efficient and timely manner--Valid CA driver's license with ability to transport self to daily appointments in the communityPREFERRED:--Use of Spanish language skills in a professional context--Previous experience working with adolescents and young adults with physical or mental health difficulties and their families (such as volunteer in hospital or clinic, experience in community program providing such supports or services)--Public speaking experience--Experience with electronic medical recordsSPECIAL REQUIREMENT - THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT
