Nemours is seeking a BBMT Physician Assistant/Advanced Practice Nurse to join our team in Wilmington, DE.
In this role, the clinician will provide specialized care and manage pediatric bone marrow transplant patients, along with a multidisciplinary team including nurses, research coordinators, pharmacists, social workers, nutritionists, child life specialists, therapists, consultant medical groups, administrators, pediatric trainees (residents and fellows) and supervising physicians.
This Master's-prepared certified clinician will be responsible for the care of a defined patient population and be vital in providing continuity of care for patients in both outpatient and inpatient arenas throughout the timeline of transplant. Critical analysis of the health care delivery system and clinical outcomes is essential to maximize the delivery of quality, cost-effective care for this complex patient population. This BBMT program is a FACT-accredited Pediatric Allogeneic and Autologous Hematopoietic Progenitor Cell Transplantation and Marrow Cellular Therapy Product Collection Center and a NMDP Network Transplant Center.
Description Patient Care Management: The BBMT PA/APN has both inpatient and outpatient responsibilities. Specifically, the PA/APN role includes the following:
Managing BMT outpatients, including seeing patients in the infusion center and clinic; documenting care in progress note; implementing, adjusting and communicating therapy plans for management of day therapy infusions; laboratory and other testing; reconciling medications; ensuring home care orders for infusions and nursing care are in place; and providing continuity for patients under the supervision of the BMT attending physicians.
Working with referring medical teams to assess patients prior to transplant, ensuring quality pre-transplant care (including pre-transplant eligibility orders) and transition to the BBMT service. Communicating with interdisciplinary team patient's specific needs.
Ensuring quality care for BBMT inpatients, including admission orders, apheresis orders, chemotherapy preparative regimen and GVHD prophylaxis orders, additional transplant-specific supportive care orders, reconciliation and prescription of discharge medications, and communicating with the multidisciplinary team.
Responsible for assuring compliance with clinical research studies applicable to BBMT patient population. Working closely with research staff to ensure all research-specific studies are ordered and followed up on.
Responsible for assuring compliance with FACT and CIBMTR standards and registry. For example, tracking transplant-related morbidity and mortality, as well as type and length of treatment for morbidities.
Developing continuous quality improvement projects and tools to optimize patient care delivery.
Together with informatics, Epic Beacon team and chemotherapy safety team, working to ensure all BBMT order sets are accurate, up-to-date and compliant with clinical practice guidelines.
Vital role in annual re-evaluation of Clinical Practice Guidelines and ensuring implementation in day-to-day practice. Working together with quality coordinator, multiple disciplines and sub-specialties to ensure guidelines meet FACT standards.
Identifying needs and developing tools to improve patient care.
