Responsibilities: Provide proactive intervention and care coordination, to include treatment planning, assessment, monitoring, educating and referrals to individuals in identified special needs populations that are at risk or high cost to ensure that these individuals receive optimal care across the continuum. Ensure direct individual care coordination to enhance patient opportunities to manage health risks and/or health condition, advocate and empower patients to successfully navigate health across the continuum of care.
Requirements: Current Registered Nurse in the State of Michigan or MSW. BSN or BS in a health related field required within one year of hire; or MSW. Care Management Certificate and/or obtainment within 2 years preferred. Minimum of one year in the area of specialization and a minimum of three years of experience in patient education, nursing, planning, case management, or similar experience. Experience in a leadership capacity preferred. Ability to use assessment, planning, implementation and evaluation skills for effectiveness of patient outcomes. Ability to organize, prioritizes, and manage time. Knowledge of experience in utilization management and/or discharge advocacy preferred. Excellent interpersonal and communication skills. Analytical abilities along with sound documentation capabilities are required to educate physicians, nursing staff and other hospital departments in the continued care planning process.
