The Vietnamese Association of Illinois (VAI) Senior Health Services (SHS) Program Manager will provide intensive case management services to senior clients of our Community Care Program (CCP) to ensure that they are connected to health services such as primary care, mental health and dental services. The SHS Program Manager will serve as a point of contact for clients, partners, CCP managers, and other VAI staff.
Primary responsibilities include coordination of access to health services, partnering with Federally Qualified Health Centers to reduce barriers to service, providing Vietnamese interpretation and/or translation, outreach initiatives and follow up to clients to ensure quality of services provided.
A competitive candidate will have bilingual (English/Vietnamese) fluency to serve as a point of contact for our limited-speaking English clients, our Federally Qualified Health Center partners and VAI staff. Due to the diversity of our client base, this candidate must be able to work with clients from various cultural and ethnic backgrounds.
VAI will prioritize candidates with a Bachelor of Science in Health and/or M.A. in Healthcare. VAI will enroll the SHS Program Manager in Heartland Alliance’s Cross-Cultural Interpreting Services (CCIS) program. The Senior Health Services Program Manager will report directly to the Director of Program Services and collaborate closely with Community Care Coordinators and the Health Program Coordinator to increase the number of clients served each month.
Assist senior clients in navigating the health delivery system, gaining access to appropriate services and community resources and coordinating the transitions between health providers
Collaborate with federally qualified health centers to coordinate access to health care
Provide outreach and engage with clients by eliminating social, cultural, linguistic and educational barriers to promote and encourage self-care
Assist clients in utilizing community resources, including Medicare/Medicaid
Ensure clients receive timely access to appropriate services by promoting and modeling advocacy skills
Participate in all staff development trainings and meetings
Complete all documentation in a timely manner
BA/BS Degree in social science, social work or related field; 2 years’ program experience in a social service capacity
Bilingual (English/Vietnamese) required
Understanding of the medical and behavioral health conditions when identifying clients’ needs and communicating with providers
Ability to work with clients from various cultural and ethnic backgrounds demonstrating cultural knowledge and sensitivity to populations being served
Excellent time management skills and attention to details to maintain focus and meet case load expectations that are essential to this position
