Rutgers, The State University of New Jersey is seeking a Habilitation Counselor for the Department of Integrated Case Management Services at Rutgers University Behavioral Health Care.
The purpose of the Habilitation Counselor position is to provide community based outreach targeted case management services that include assessment, treatment planning, linkage and monitoring.
Provides targeted case management services to assist individuals to gain access to needed medical, social, education, mental health, addiction, co-occurring and other services.
Develops, monitors and revises Individualized Treatment Plans based on the assessment, as well as the preferences, strengths and expressed needs of the individual served to address the medical, social, educational and other services needed.
Links individuals served to needed services including benefits, medical, dental, mental health, co-occurring services, self-help, employment, legal and housing.
Monitors activities and contacts that are necessary to ensure that the Treatment Plan is implemented.
Helps individuals identify triggers for relapse and focus on a lifestyle centered on recovery.
Assists individuals with adherence to medication protocols.
Demonstrates the ability to engage and establish a working relationship with, and provide services to individuals who are diagnosed with a serious and persistent mental illness and co-occurring disorders, who present engagement challenges.
Demonstrates the ability to monitor the individual, identify crisis and contact a supervisor for direction.
Manages termination / discharge issues in an effective and timely manner.
Collaborates effectively with members of the department, and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for consumers and families.
Documents services in compliance with the Joint Commission, state and UBHC standards; and provides content that reflects the quality standards of UBHC's clinical record policy.
Regularly attends and participates in in-service meetings and other designated training events that will enhance skills, as documented by continuing education reports.
Performs other related duties as assigned.
Home Location Campus: Piscataway (RBHS)
Experience with community based service provision to adults with psychiatric disabilities preferred.

Must have the ability to focus on the task at hand, despite numerous interruptions.

Must possess own vehicle or alternate means of transportation for use to outreach individuals served.
