Providence is calling a Registered Nurse (RN) Case Manager (Day Shift, 1.0 FTE) to Providence ElderPlace in Seattle, Washington. In this position you will:
Facilitate interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities.
Collect the necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well being of participants.
Act as a medical advocate for the participant, and coordinate care services with the Interdisciplinary team.
Build strong relations with participants, caregivers, families and community providers so that communication and coordination occurs in a manner that facilitates optimal care and financial oversight.
Required qualifications for this position include:
Graduate of accredited school of nursing
Current Washington State RN license
Current Washington State drivers license
Current CCM, NACCM or ACME upon hire or must be obtained within 1 year of employment
Proof of auto insurance
One year of long term care or geriatric nursing experience
Preferred qualifications for this position include:
Bachelor Nursing Degree
Current Cardiopulmonary Resuscitation (CPR) certification
Medical Surgical Experience or Rehabilitation Medicine
