The Tuberculosis (TB) Public Health Nurse provides directly observed therapy (DOT), and case management services for individuals who live within the city limits of Chicago, and have been identified as TB cases and/or suspects. This position requires on-going communication/collaboration with all clinicians involved in patient care in addition to Chicago Department of Public Health (CDPH) and Suburban Cook County staff to facilitate proper TB management and care. The Public Health Nurse (PHN) will conduct physical assessment/triage in sputum collection, and referrals as necessary to ensure continuity of medical care. The PHN will receive assignments from and collaborate closely with the TB Nurse Case Manager, and participate in weekly case conferences.
Completes a thorough initial case management intake assessment with each newly assigned case to determine treatment plan and initiate directly observed therapy (DOT).
Communicates with Chest Clinic administration to schedule patients for outpatient follow-up visits.
Continually assesses and communicates both verbally and in Cerner progress/status of TB patients receiving DOT for compliance, adverse reactions to medications, etc.
Conduct home visits, complete physical assessments, administer DOT including intramuscular injections, provide phlebotomy services as needed, and etc.
Deliver specimens to laboratory for processing.
Maintain communication with CDPH TB central office by faxing patient interview sheets, clinical notes, prescriptions, labs, diagnostics, etc. as needed.
Initial and data entry updates (labs, treatment changes, DOT numbers, etc.) on each assigned patient in I-NEDSS as well as Cerner (scanning of documents from outside facilities into Cerner)
Enter monthly case management notes into Cerner
Attendance and reporting of cases at weekly case conference at CCHHS as well as monthly and quarterly cohort case conferences with CDPH.
Completion of appropriate paperwork/communication for patients requesting inter-jurisdictional transfers as needed.
Contact/follow up with patients who miss medical appointments
Facilitates/processes specialty labs i.e. drug levels
Assists with prescription refill process for Chest Clinic patients as needed.
Provides comprehensive case management services to clients as it relates to public health and decreasing health disparities by retaining clients in medical care.
Works closely with the multidisciplinary team on issues surrounding patient referral and patient service plan, both to include referrals internal and external.
Conducts nursing/TB assessment for immigrants/refugees scheduled in Chest clinic
Utilizes evidence based practice and keeps abreast of local and federal policy as it relates to TB patient care and public health
Follows all hospital/medical-setting standards as defined by JCAHO/TJC
Maintain professionalism at all times while on duty
Maintain all levels of communication daily
Serves as acting TB Nurse Case Manager in her absence
Extensive knowledge of core public health principles and practices
Demonstrated capacity for public health advocacy
Proficient in Microsoft Word, Excel, etc.
Demonstrated ability to clearly and professionally communicate information to a variety of individuals, and across many cultures and audiences both orally and through written documents.
Ability to work independently.
Excellent communication and organizational skills required.
Ability to travel with own vehicle anywhere within the city limits of Chicago.
Ability to wear respiratory protection as required.
