Pomona Valley Hospital Medical Center Palliative Care Nurse Navigator
Pomona Valley Hospital Medical Center (PVHMC) is a 437-bed, fully accredited, acute care hospital serving eastern Los Angeles and western San Bernardino counties.
The nationally recognized, not-for-profit Hospital is home to four Centers of Excellence: The Robert and Beverly Lewis Family Cancer Care Center, Stead Heart and Vascular Center, Women's & Children's Center, Sweet Success and Regional Kidney Stone Center, and offers a full-service Emergency Department, Neonatal Intensive Care Unit and Perinatal Center. Specialized services include sleep disorder diagnosis and treatment, robotic surgery, and the Family Medicine Residency Program, in affiliation with the University of California, Los Angeles.
Position Summary: The nurse navigator is responsible for navigating the patient and significant others through the continuum of care. The navigator will develop an advocacy relationship with the patient and patient's physicians to ensure timely and cohesive diagnostic and therapeutic interventions for Palliative Care. The navigator acts as an educator and liaison for the patient, families, community, associates and other healthcare professionals. The navigator develops and standardizes educational materials for patients. The navigator provides support and helps trouble shoot needs as they arise. May perform other duties as assigned.
Job Qualifications [Required] Must have current and valid license as a Registered Nurse issued in the state of California. Bachelor's degree. Three (3) years of professional clinical experience in Palliative Care program specialty.
[Preferred Qualifications] ELNEC or Palliative Care Certified.
