Maryland MENTOR is seeking an IntakeCoordinator in our Baltimore office to match potential mentors with clientsutilizing clinical principles, monitor the matching process, arrange neededservices upon discharge and maintain ongoing contact with placement referralsource.
Conducttelephone intake with follow-up assessment interviews on appropriate referrals.
Ensuresthe receipt of a referral packet.
Securesfunding information and verifies funding/insurance coverage arrangements.
Securesall services that will be needed by the client when placed.
Presentsclinical assessments with preliminary treatment plan recommendations to ProgramManager and/or Clinical team, in consultation with psychiatric or medicalconsultant as needed.
Implementsthe use of clinical principals to match clients with potential mentors. Meetswith Program Manager, Clinical Supervisor and/or Recruiter to review potentialclient-mentor matches and assists in case assignments.
Completesall placement paperwork.
AssistsCoordinators in locating appropriate homes when disruptions occur.
Maintainsongoing relationships with funding agencies and develops new resources throughmarketing presentations.
Ensuressufficient amount of referrals by marketing services with funding sources.
Bachelor'sdegree in human services field or equivalent in education and experiencecombined
Licensed Social Worker (LBSW, LGSW, LCSW)
Two yearsof related experience
Currentdriver's license, car registration and auto insurance
