UWMC has an outstanding opportunity available for a part-time 80% SOCIAL WORKER to provide services for patients and families receiving care through the UWMC Regional Heart Center. This Social Worker will serve as a member of the Regional Heart Center Social Work & Care Coordination team, as well as assisting other inpatient teams and services. This social worker's duties include conducting psychosocial assessments with patients and families to help determine candidacy for advanced heart failure therapies, planning and implementing discharge plans, helping patients/families experiencing loss, and working collaboratively with interdisciplinary teams on tangible and behavioral issues affecting patient ability to adhere to care and proceed to a safe discharge. Qualities desired include strong assessment skills, ability to work collaboratively with other disciplines and programs to coordinate care, and skills/experience needed to ensure safe and comprehensive transitions for patients. This social worker also covers Saturdays as part of a general inpatient social work team. This position reports to the UWMC Social Work & Care Coordination department, and receives ongoing supervision as well as continuing education opportunities on-site.
Primary Job Responsibilities:
Provide direct social work services: screening, psychosocial assessment, interdisciplinary care planning, support services and discharge planning to patient population.
Provide interventions to target population including patients with chemical dependency and mental health issues.
Assist patients needing general resources including transportation and housing and referrals to community resources.
Provide telephone information and referral to individuals and agencies calling for information or consultation
Provide informal orientation/training to interdisciplinary staff and providers.
May be hired as a Social Worker 1 or 2, depending on candidate's qualifications. Social Worker 2 participates as a Practicum Instructor for the School of Social Work as assigned.

A Master of Social Work Degree from a program accredited by the Council of Social Work Education AND six months of experience in a health care delivery system OR equivalent education/experience. Health care experience can include a practicum placement as part of a social work training program and/or employment experience.
Legal Requirement - Licensed as an Associate LICSW or LASW within 90 days of employment.
