IndianaMENTOR offers home and community-based services to children and adults withintellectual and developmental disabilities, as well as autism spectrumdisorders, mental health challenges, physical disabilities with complex medicalneeds and brain injury. With program locations throughout the area, IndianaMENTOR offers a range of residential services with supports tailored to theneeds of each individual as well as hourly support and respite servicesdesigned to allow each individual to be as independent as possible.
Essential Job Functions
Administers thereferral process. Works with clinical and operations team to facilitate clientplacement and obtain appropriate services for clients.
Collects,verifies, records and processes client demographics, insurance/payment andreferral information.
Inputsdata and completes all placement paperwork. Identifies problems andresolves issues preventing successful documentation/entry.
Securesfunding information and verifies funding/insurance coverage arrangements.
Completesall client reporting requirements consistent with corporate and regulatorystandards
Serves as a resourceto others in problem resolution. Applies knowledge and understanding ofprograms, policies, procedures and information to assist others in resolvingadministrative issues within scope of authority.
Assistswith emergency coverage and crisis intervention support for all clients asneeded
Performsother related duties and activities as required
Bachelor'sdegree in human services or related field and two years' experience preferred.
Anequivalent combination of education and experience which provides proficiencyin the areas of responsibility listed above may be substituted for the aboveeducation and experience requirements.
