**CCN - Continuity of Care Program** Current openings area: *100% days. *100% nights. *per diem. The Continuity of Care Nurse (CCN) coordinates all aspects of care leading to a timely transition to the next level of care for patients on acute care units. The CCN serves as a liaison to patients, families, medical teams, nursing staff, social services, interdisciplinary team members, consulting and referring physicians and agencies involved in the care of patients from admission through the discharge process. The CCN identifies the clinical targets and milestones for discharge and eliminates barriers as needed. The CCN communicates the discharge plan to appropriate team members.Requires min. two years acute care or critical care nursing experience.
**APA (Ambulatory Procedures Area)**
There are currently two positions open within APA (Ambulatory Procedures Area). *********** *Lipid Apheresis/APA - 60% Day shift position open, 8 hr shifts.
The APA Lipid Apheresis (LA) nurse would provide a dialysis type treatment to a select group of individuals with very high cholesterol on a weekly or bi-weekly basis. The La nurse is responsible for the set up and break down of the treatment machine, placing IV access, monitoring vital signs at designated intervals, charting in ORCA and ordering all supplies needed. On days/shifts with no LA patients, the LA nurse will work in the Ambulatory Procedure Area assisting with the care of a variety of outpatient Radiology procedural patients and infusion patients.
Responsibilities Specific to Lipid Apheresis
Performs LDL apheresis procedure for HMC out-patient patient population.
Assesses patients' medical history and clinical condition for procedural concerns, risks and contraindications to LDL apheresis.
Obtains laboratory blood samples, reports critical lab results to medical providers and manages corrective action per orders.
Monitors the patient response to the procedure and make technical and medication changes appropriately.
Communicates with LDL Apheresis medical provider to address potential and immediate clinical concerns.
Adheres to unit policies and procedures compliant with regulatory requirements for LDL Apheresis documentation.
Functions independently and is responsible for daily workflow management, scheduling of patients into Epic, environmental safety and interfacing with other departments.
Assist in policy and procedure development, review, revision and implementation, as needed.
ICU experience, lipid apheresis experience, dialysis experience desirable. ******************* *APA - 60% Day shift position open, 12 hr shifts.
Provide pre and post care for outpatients receiving a variety of outpatient radiology procedures. Provide IV infusions/transfusions for outpatients. Provide light to moderate sedation care during bone marrow biopsies and lumbar punctures. Assist heme/onc clinic with blood draws, monitoring lab results, performing injections, various patient care coordination.
Per diem/on-call position open (for taking call M-Th, 8pm - 7am).
Requires either ICU experience or Endo./GI experience.
Per diem, variable shifts position open. Min. two years ICU experience required. PACU experience desirable.
**ASU/Ambulatory Surgery Unit**

**Inpatient Psychiatry** * Per diem position open - 5MB - Psych ICU. * 80% Day and 60% Evening positions available; Per diem positions open - 5WA - Acute Psych. * Per diem positions open - 5WB - Acute Psych.
**Foot and Ankle Clinic (Ninth and Jefferson location)** 100% Day shift position. Requires one years of nursing work experience. Ambulatory Care experience preferred.
**After Care Clinic and Pain Relief Services** 100% Day position available. RN helps patients with pain management that includes both the acute/transitional pain service as well as consultation and referral to internal and external providers for chronic pain. The RN2 will assist with pt teaching, care coordination, procedures and more. Familiarity with working with under-served populations ( pt's experiencing chemical dependency, homelessness, pain management (transitional, chronic, etc), pain mgmt procedures and care coordination.
**Adult Medicine Clinic & (Tuesday Evening Clinic, 12-8:30)** 90% Day shift position. Requires one years of nursing work experience. Ambulatory Care experience preferred.
**Diabetes Clinic** 60% Day position available. This clinic provides diabetes education for effective self-management, blood glucose monitoring, help with meal and exercise planning, as well as guidance about lifestyle changes that may have positive effects on diabetes, medication and insulin therapy, routine health management to check for kidney, eye, skin, heart, thyroid and circulation problems, prevention and treatment of diabetes-related complications, including specialists in kidney problems, training in using a continuous glucose monitoring system for patients of the clinic and diabetes-related mental health evaluations and treatment. Requires at least one year of RN work experience.
**Madison Clinic** 80% Day shift position. Requires one years of nursing work experience. Strong med/surg assessment skills. Strong IV and wound care skills. Experience working with underserved populations and with HIV patients.
**Pioneer Square Clinic** 90% Day shift position. Requires one years of nursing work experience. Experience working with people who are homeless, mentally ill, substance abuse, marginalized with complex medical and social needs. Working knowledge of trauma informed care, harm reduction, motivational interviewing and standards of care for people without housing. Good communicator and collaborator with other professionals inside and outside of Harborview Medical Center. Knowledge of community resources for vulnerable people in the Seattle/King County area. Experience working with diverse populations including limited English speakers, LGTBQ, immigrant/refugees, and people of color.
**Pioneer Square Clinic: Health Care for the Homeless** 90% Evening (Monday - Friday) position available. Requires one years of nursing work experience. Experience working with people who are homeless, mentally ill, substance abuse, marginalized with complex medical and social needs. Working knowledge of trauma informed care, harm reduction, motivational interviewing and standards of care for people without housing. Good communicator and collaborator with other professionals inside and outside of Harborview Medical Center. Knowledge of community resources for vulnerable people in the Seattle/King County area. Experience working with diverse populations including limited English speakers, LGTBQ, immigrant/refugees, and people of color.
**Pioneer Square Clinics (will work at dual locations)** 63% Day (Monday - Friday) position available. This position has two work locations. One in permanent housing complex for men who are formally homeless and chemically dependent and the other in small 2 room low barrier clinic serving people who are homeless, unstably housed, marginalized and reluctant to use traditional community services. This position may require occasional work in other community based settings. All locations require a focus on outreach and engagement building relationships with people in order to engage them into medical care. The nurse provides: triage, chronic disease education, phone and in-person care coordination,transitions of care,acute medical care, completes medical orders and support chronic disease management.
**Ambulatory Care Services/Outpatient Care Manager/Care Coordinator** Full time positions available. And, per diem position available. Care management, care coordination, and transition management for patients with recent hospital or emergency room utilization, chronic conditions, multiple co-morbidities, and social, financial and cultural barriers. Also works with Accountable Care Network and population health approaches. Under general supervision, this RN will collaborates with the interdisciplinary team to identify, engage, assess, plan, implement, coordinate, monitor, and evaluate options and services in meeting the needs of high risk individuals. The RN will work closely with clinic(s) and function across the continuum of care, interfacing with patients, care givers, providers, and community resources to facilitate the best possible clinical outcomes and optimal level of wellness while using resources appropriately. 2 years RN work experience required; outpatient clinic experience desirable. Care Coordination/Transition Management (CCTM) certification (or equivalent) desirable.
**Outpatient Mental Health Services (OBOT Program)** 100% Day shift position available. Outpatient RN Care Manager for the OBOT program. The OBOT Program offers medication assisted treatment to adults with opioid use disorders in a behavioral health clinic setting. Experience working with patients with addiction is required, current BLS certification in CPR.
**Medical Respite** 90% Rotating/Variable position available; Per diem position available. Experience in an inpatient setting is preferred with two years of RN experience.
**Eye Institute** 100% Day shift positions and per diem position available. This Eye Institute clinic RN position focuses on the clinical support of the Oculofacial surgical practice. In collaboration with the Oculofacial Patient Care Coordinator (PCC), the RN will assure seamless coordination of care in both the pre-surgical and post-surgical phases and assist physicians with clinic based procedures and telephone triage .The ideal candidate must be well organized, have excellent communication skills and be resourceful to facilitate meeting patient needs.
**Eye Institute (Operating Room)** Per diem position available. Experience with Surgical Specialties prefer experience in ophthalmology. Experience in an OR -Circulating/Scrubbing. Experience with Moderate Sedation and this position will also be required to make post op phone calls.
**Kidney Clinic** 100% Day position available. RN experienced preferred.
**Orthopedic Spine Clinic** 100% FTE day shift position available. 2 years working as a RN required.
**SANE Nurse position (Sexual Assault Nurse Examiner)** Per diem position available. On call position providing care to adult, adolescent and pediatric victims of sexual assault including physical exam and forensic evidence collection. Service currently is provided at Harborview Medical Center and Valley Medical Center at a minimum. Program may expand in the next year to cover additional facilities in the Seattle area. 40 hour didactic SANE training is provided along with extensive orientation. Minimum availability of 72 hours per month in 12 or 24 hour shifts. Must be able to arrive at the hospital within 40 minutes of receiving page. Minimum 2 years nursing experience required. Emergency department, Women's health and/or Pediatric experience preferred.
**UW Medicine - Community Care Line/Nurse Advice Line/Transfer Center** (This position is located in an office at Boeing field).
Full time, variable shifts position available. Also, per diem, variable shifts position available. ER; outpatient; telephone triage experience desirable.
