Seaside, Oregon – Providence Health & Services is seeking an excellent on-call nurse practitioner to join our innovative, comprehensive care and social services program, Providence ElderPlace. We are seeking a primary care clinician to see participants in a variety of community-based settings including clinic, assisted living, memory care, and private residences. · Current, unencumbered Oregon Nurse Practitioner (NP) license
· Prescriptive drug privileges and active DEA number
· One year experience with a frail or elderly patient population, preferred
· Education and/or experience in geriatric health care, preferred
· Knowledge of geriatric medicine
· Ability to conduct thorough health status assessments resulting in appropriate health care treatment plans
· Ability to assess medication regimen for accuracy and efficacy
· Ability to work in an interdisciplinary team setting
· In compliance with the Department of Human Services (DHS), criminal background check is required for this position, based on assigned duties. Fingerprinting may also be required
