The Community Outreach Nurse will be responsible for assisting individuals and families to develop the skills, knowledge, and attitudes necessary to manage long-term chronic illnesses. The candidate will work collaboratively with physicians, and other members of the health team to navigate access to resources in the community or health system to underserved/uninsured populations. The outreach nurse uses critical thinking to manage complex patients, perform health assessments, provide care coordination, make/receive medical referrals and participate in community health screenings.
The ideal candidate must have excellent communication skills, oral/written in English and Spanish, is sensitive and respectful of diverse/underserved populations, can adapt to new challenges and be a great team player. Associates Degree in Nursing required; BSN or MSN preferred. The ideal candidate is proficient with Epic, data analysis, Microsoft Office, and grant writing.
The schedule is primarily Monday through Friday business hours. Participation in community events on weekends required as needed.
