This position is responsible for completing psychiatric assessments, developing clinical interventions, and coordinating case management services for mentally and emotionally distressed patients in a mental health setting. Responsible for assisting and supporting behavioral health professionals and other healthcare team members completing routine assessments.
Triages and assesses patients current level of acuity, risk and psychological functioning using effective patient enragement skills to collect information.
Identifies current patient needs in collaboration with all members of the healthcare team, and develops a plan with appropriate and available resources.
Documents all clinical interventions including de-escalation skills in the patient medical record ensuring timely submission and accurate and complete discharge plans.
Provides advocacy, assistance and support to patients and families utilizing services in the unit. In the case of suspected abuse/neglect/exploitation, documentation and reporting are completed as required and directed in policy.
Functions in a liaison role between facility, system, other health care agencies, and the community by identifying resources that may be useful, available, and/or needed.
Maintains current working knowledge of Medicare, Medicaid and other program benefits and criteria.
Requires a Bachelors of Social Work or related degree.
Requires a proficiency level typically achieved with 3 years experience as required by work unit. Banner Registry and Travel positions require a minimum of one year experience in an acute care hospital setting (experience must include working in an acute care setting within the past 12 months as a Social Worker in the specialty area).
Must have knowledge of government/community resources such as Medicare, Medicaid, long-term care or any other applicable resources/services. Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, human relations skills and time management skills. In acute care, must be able to work flexible hours and take rotating call after hours.
Basic Life Support (BLS) certification preferred.
Additional related education and/or experience preferred.
