3. No written warning or Performance Improvement Plan (PIP) within the last six months unless their manager receives approval from HR; and
4. Continuous sustained improvement is demonstrated.

Description: Centura Health Physician Group has a wonderful opportunity for a full-time advanced practice psychiatric Nurse Practitioner or Physician Assistant. The position provides outpatient psychiatric evaluation and medication management services at our medical office building located on Porter Hospital Campus. The right candidate will join an established practice that focuses on professional excellence, patient relationship, treating the whole person, body, mind and spirit while embracing preventive medicine. Education Requirements (i.e., BSN, High School Diploma, GED)
BSN from accredited school of nursing
Master's Degree in Nursing or applied area of knowledge or Physician Assitant schooling
Advanced Practice Registered; Prescriptive Authority required
Minimum of one year direct behavioral clinical experience in a healthcare facility or setting. Competence with DSM-V diagnostic criteria and psychiatric evaluation and medication management experience required.
License/Certifications (i.e., NP/PA, RN, BLS, ACLS)
Current Colorado Nursing License
Clinical Nurse Specialist certification in Psychiatric and Mental Health Nursing
