Family Support Worker (Home-Based Programs)

The Family Support Worker in Gads Hill Center’s center-based and/or home-based early childhood programs is responsible for providing comprehensive support to families. Family support workers are expected to develop a relationship with parents and/or guardians during the enrollment process and support parents in setting and achieving ambitious goals, as well as accessing resources to meet their family’s needs. In addition, this position is responsible for achieving all assigned milestones established by the grantee in the areas of health, mental health, family and community partnerships, disabilities and ERSEA. Family Support Workers play a key role in engaging parents in leadership roles and empowering parents as advocates for their families.
DUTIES AND RESPONSIBILITIES:
- Conduct assigned components of enrollment, including entry of accurate data into COPA and family assessment.
- Participate enthusiastically in the recruitment and outreach process.
- Refer families to resources according to needs. Document referrals and progress toward goals, and follow up with families regarding referrals on a regular basis.
- Help parents access consistent health care in a “medical home” for their children, and monitor health records in collaboration with the health coordinator.
- Maintain updated and consistent data in COPA, including all family partnership agreements, needs, strengths, referrals, etc.
- Monitor all component data on a regular basis and follow up with families missing documentation on a regular and escalating basis. Use phone calls, home visits, and any other necessary means to contact families and obtain missing items; document all efforts.
- Serve as the liaison between parent committee and Support Services Coordinator, and/or other designated staff. Plan and implement all aspects of monthly parent meetings.
- Participate in and assist with the Head Start/Early Head Start self-assessment, site monitor visits, developmental screenings and referrals to Chicago Public Schools or Early Intervention.
- Identify potential community agencies, clinics, dentist’s offices and other resources for partnerships and support establishment of linkage agreements.
- Maintain community resource guide to assist parents in identifying and accessing local community resources.
- Coordinate parent involvement activities through recruitment and organization of events and services.
- In collaboration with partner site staff and parents, ensure involvement in parent policy committees and parent meetings, and participate as needed.
- Conduct classroom/socialization observations to assist mental health providers and education staff with referrals. Attend regular team meetings with assigned classrooms or home visitors.
- Work with education staff, mental health consultants, EI/CPS staff and other pertinent individuals to meet the needs of children with special needs. Facilitate the referral process to Early Intervention or Chicago Public Schools, and provide support to families throughout the referral process. Aggressively pursue the best outcomes for children and families.
- Perform record-keeping and other reporting functions as required by Head Start Performance and NAEYC Standards and submit all agency reports in a timely manner.
- Work with the Support Services Coordinator to facilitate and ensure parent participation in the committee structure of the Head Start/Early Head Start delegate agency and city-wide council.
- Attend pertinent Head Start/Early Head Start and CPS committee meetings and trainings related to responsibilities above.
- Serve as a Certified Safety Seat Technician and support the implementation of a Child Passenger Safety program at Gads Hill Center’s sites and at community events and locations.
- Conduct monthly home visits with assigned families. This will be done with a Home Visitor during 90-minute scheduled visits. The last 30 minutes should address any issues that pertain to Family Support.
- Help organize and conduct parent groups with guidance from the Support Services & Disabilities Coordinator.
In addition to the responsibilities listed above, all Gads Hill Center employees are responsible for the following:
- Participate in agency outreach and recruitment events, including evenings and weekends.
- Participate in supervision meetings and workshops within the agency and outside (if applicable).
- Participate in civic engagement activities that advance the mission of Gads Hill Center.
- Advance parent leadership development.
- Perform other duties as assigned.
- Bachelor’s degree in Social Work or related field.
- A minimum of two years of experience working with children and families.
- Experience with multicultural programming preferred.
- Bilingual Spanish/English preferred.
- Must have a valid Driver’s License, access to reliable insured transportation, and willingness to travel locally.
- Required to obtain National Safety Seat Technician certification upon being hired, and maintain certification during employment.
- Excellent interpersonal communication and organizational skills.
- Ability to pass a criminal record and child abuse/neglect registry check.

Entry Salary Range: $30-32,000
