We are now hiring for a full-time Social Worker/Counselor to join our Project Access Team!
The Social Worker/Counselor will provide community and home-based clinical services provided to clients experiencing acute and chronic presentations of mental illness.
Essential Job Functions:
Provides assessments, treatment planning and counseling, crisis stabilization, and case management. Some clients also present with substance abuse problems.
Provides comprehensive mental health assessments and develop recovery oriented individualized treatment plans specifying service array.
Provides individual and group therapy, family education and support activities, individual skill building and case management services for outpatient clients.
Services are provided in the office, community and some work is conducted in the client’s home.
Some evening hours required.
Master’s Degree in Social Work or Counseling required.
LSW or LCP required
LSCW or LCPC preferred
Bilingual (English/Spanish) preferred.
Must have excellent verbal and written communication skills.
This position requires travel and evening hours to provide home-based services to families.
Must have access to a vehicle, proof of insurance and valid driver’s license to perform essential job functions.
Ability to work effectively with diverse individuals and groups.
