TheUniversity of Maryland, School of Nursing is recruiting for a part-time temporary Health CareProvider/Nurse Practitioner.Theselected candidate will practice 1-2 days per week including providing primaryand secondary specialty care in an ambulatory care based practice setting at Prince Georges Hospital Center's (PGHC)Ambulatory Care facility.
Collaboratewith the health care team to direct the day-to-day clinical activities andproviding primary care services in collaboration with an interprofessional teamcomprised of pharmacist, social workers, lawyers and with other healthprofessionals.
Ensurethe availability and accessibility of clinical services to patients referred tothe Interprofessional Transition of Care ITOC clinic at PGHC.
Provide clinical assessment, diagnosis, treatment and management of chronic, stable morbid and acute common short-term Illnesses.
Provideadult interventions including physical examinations, routine screenings per theEPSDT guidelines, medical interventions.
Orderdiagnostic interventions as necessary for diagnosis and management of clinicalconditions. Interpret results and design and implement interventions to affectappropriate and optimal care and attain improved health outcomes.
Makereferrals to other providers and specialists, such as Social Workers,Physicians, and Specialists when necessary for screening procedures, diagnosticconsultation, or treatment.
Facilitatethe transfer of health knowledge within a community through continuous teachingand counseling of patients. Foster team participation in individual and grouphealth promotion and health education activities.
Participatein the formulation, implementation, and coordination of standards, policies,and procedures for the health services provided by the ITOC.
Collaboratewith pharmacists, social workers, Wellmobile Nurse Practitioners, NurseCoordinator, outreach worker and lawyers in designing an effectiveinterprofessional care plan that supportsoptimal health and medical management of this comorbid population of highutilizers. The ITOC and the Wellmobile will work in concert to address healthneeds and social determinates of health in this complex patient population.
Participatein research and evaluation activities at the request of the Community HealthResource Commission Grant under the direction of Associate Dean for PharmacyPractice.
Participatein quality assurance and peer review activities for continual monitoring andimprovement in clinical services.
Preceptentry-level and graduate community health in care management and graduate NPstudents in all areas of NP functioning in the ITOC.
Actas a liaison between representatives of the communities served and theUniversity of Maryland; assists with the management of change within theprogram whenever necessary.
Providefeedback on a continuing basis to other team members and other staff on theirperformance relative to clinical management and interprofessional teamperformance .

Completion of an accredited Mastersprogram and current licensure as a Nurse Practitioner.
Doctorate in Nursing Practice (DNP) Degree preferred.
Requires certification by the American Academy of Nurse Practitioners Certification Program.
May require training relatedto occupational safety and health, environmental compliance, shipping ofhazardous materials and/or ionizing radiation as well as electronic health record.
