We have an exciting opportunity to join our team as a Patient Flow Coordinator (RN).
In this role, the successful candidate Responsible for organizing, prioritizing and coordinates activities related to the placement of patients throughout the institution
- Screens and facilitates all patient admissions prior to admission ER department, PACU, hospital to hospital transfers, direct admits and elective admissions) utilizing established guidelines, policies, procedures and Milliman Care Guidelines for medical necessity. Assesses potential candidates for transfer or admission to Tisch by collecting and reviewing all clinical and financial data necessary, from patients and their representatives, NYU and external physicians, hospitals, third party payers, other agencies as appropriate. Makes recommendations for alternative settings when inpatient admission is not indicated. Refers patients to Social Work/Discharge Planning Unit as necessary for financial assistance or anticipated discharge planning issues, during admissions assessment process. Accurately and thoroughly documents patient assessment on forms and/or in hospital information systems, as needed, with specific emphasis on indications or barriers to admission, allowing for appropriate follow-up and admission decisions by others, when necessary.
- Prioritizes placement of patients through accurate nursing assessment and knowledge of individuals patient care needs. Maintains data and prepares reports related to the outcome of pre-admission assessment activities and decisions, and outreach efforts. Identifies problems and trends, and develops recommendations for corrective actions.
- Appropriately assigns the correct level of care for all admissions based on the medical information and physician assessment.
- Actively participates in the daily bed huddle to facilitate house-wide movement of patients.
- Coordinates transfers of patient transfers of patient internally and externally to facilitate placement of new admissions.
- Review status of patients in collaboration with staff and negotiate final determinationabout patient placement that is consistent with patient care standards, hospital goals andobjectives and regulatory requirements.
- Engages in on the spot education of clinical staff about the best use of systems thatenhance efficiency in patient flow.Provide education to the medical staff and physicians offices staff relative to theadmission process.
To qualify you must have a Bachelor's Degree in nursing or related field. Licensed as a professional registered nurse in the State of New York. Minimum five years of clinical practice in acute care setting. Requires strong clinical, communication and interpersonal skills, with the ability to work effectively and collaboratively with all professional and support staff.Demonstrate leadership abilities. Strong organizational and prioritization skills. Knowledge of utilization management and quality improvement. Requires strong organization and problem-solving / issue resolution skills and engage other disciplines in the process.Demonstrates high level of initiative and self direction, flexibility, and ability to adapt to change.Capability in word, excel, and related programs. Physical stamina, manual dexterity, visual, and aural acuity to perform responsibilities..
Required Licenses: Registered Nurse License-NYS
Master's Degree in Nursing, health-related field or management.
