The Valve Clinic Coordinator is responsible for coordinating referrals and patient visits to the Valve Clinic. The Valve Clinic Coordinator utilizes discretion and independent judgment to facilitate & establish seamless care for patients who are candidates for trans catheter valve therapy from time of referral through pre and post procedure evaluation and follow-up. The Valve Clinic Coordinator applies the advanced nursing process to manage this subset of patients and coordinates numerous aspects of the Valve Clinic program while facilitating a team approach and collaborating with the multidisciplinary Valve Clinic team. This position requires marketing and communications with off-site local and out of town facilities to promote and coordinate the Valve Clinic Program. In addition, the Valve Clinic Coordinator provides for optimal secondary preventative cardiac interventions as well. The Valve Clinic Coordinator demonstrates personal accountability for relationship-based care, the organizational mission, and supports all core values, in addition to contributing to the programs strategic planning.
The Valve Clinic Coordinator will also work with CTS office on an as needed basis for both follow up outpatient clinic as well as aortic surveillance clinic. Clinical coordination in other programs which require multidisciplinary physician coordination and treatment prior to the procedure may be assigned. Will also be responsible for reporting program outcomes on a regular basis to appropriate team members as well as initiating program improvement process development based on those outcomes.
- Graduate of an accredited School of Nursing
- Advanced Practice Nursing Degree
Work Experience Requirements
- Two years RN experience required
- Two years of Advanced Practice RN experience preferred.
- Two years of work related Cardiology/Heart Failure/Structural Heart experience preferred
- Two years Marketing and/or Business Development experience preferred
- Communication and problem solving skills
- Communicates and works closely with Physicians
- Colorado ARNP Licensure
- Colorado RN Licensure
- Current Basic Life Support (BLS) certification from American Heart Association
- Current ACLS certification from American Heart Association
- Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
