Responsible for identifying appropriate candidates for home care services. Evaluates patients referred for treatment to determine appropriateness for admission. Position will provide exceptional customer service while participating in the discharge planning process and evaluating patients for agency services. Ensure referral source and patient needs are met. Completes agency referrals, providing accurate demographic information and appropriate information for care. Position will provide exceptional customer service while participating in the coordination of care/scheduling process for admissions to homecare. Problem solving through excellent written and verbal skill. Data entry and clerical duties.
Minimum Education Requirements
High School Graduate (Diploma) or equivalent required.
Degree in a health care related field preferred.
General knowledge of state and federal regulations and JCAHO standards for home health services.
Good PR/communication skills.
Advanced knowledge of computers and Windows products; Medical Terminology
Minimum Experience Requirements
1 year of home care, hospice, home oxygen, HME, or home infusion experience preferred. Prior customer service/relations and or telephone sales experience preferred. Excellent customer service skills and a commitment to meet the needs of all customers and patients. Knowledge of comprehensive home care services and supporting community organizations, strong organizational, and analytical, and communication skills preferred.Understanding of different levels of care and staffing to meet patient care needs preferred. Must also have the ability to work independently, to coordinate multiple facets of patient care, to prioritize work, and to manage time to accomplish multiple tasks in an active office atmosphere with multiple interruptions. Strong analytical, organizations, time management, and problem solving skills.

Serves as an effective liaison between Customer Care, professional field staff, , clinical leadership, Infusion business line and HC Operations staff to appropriately coordinate patient care.*
Demonstrates excellent communication skills in soliciting complete and accurate scheduling information in an efficient and timely manner and reconciles this information as well, which in turn identifies staff availability and types of visits done at the end of one day and to be done next day.*
Inputs various data into spreadsheets daily.*
Verifies services and ensures that orders, client information, and supporting paperwork, are transferred to the appropriate line of business representative.*
Participates in necessary updates and communication across all business lines and departments.*
Provides assistance in resolving systems, workflow and documentation problems. Acts as a mentor for CCR I associates.*
Demonstrates proficiency in all computer software programs required to perform duties.*
Completes data entry, filing, and paperwork accurately and in a timely manner.*
Maintains referral intake log and ensures all referrals are processed and/or accounted for at the end of each day.*
Development and maintenance of updated documentation, reference information and systems.*
Demonstrates ability to prioritize new patient Admissions and revisits and determine when they must be scheduled.*
Demonstrates outstanding organizational skills in the day to day work flow.*
Demonstrates ability to interpret referral information in order to prioritize order of discipline evaluation.*
Appropriately seeks input from clinical resources to analyze complicated cases.*
Recognizes critical information and facilitates the exchange of information between CM’s, Clinical Managers/Supervisors, and other internal departments.*
Maintains all logs in an organized manner and is able to retrieve any staffing information that is needed timely.*
