The goal of medical case management is to help clients with HIV gain and maintain access to primary medical care and treatment. In the process of meeting this goal, medical case managers must assess and facilitate each client's progress toward self-sufficiency. Medical case management is a formal and professional service that links patients with chronic conditions and multiple service needs to a continuum of health and social service systems. Medical case management strives to ensure that patients with complex needs receive timely coordinated services, which assist a patient's ability to function independently and remain in care for HIV. Medical case management assesses the needs of the client, their support system, including family and others, and arranges, coordinates, monitors, evaluates and advocates for a package of multiple services to meet the patient's needs.
Coordinate care and services HIV population. Collaborates with patient care team and develop plan of care. Identify and assess community based services. Coordinates referrals to outside sources.
Supervises Grant funded positions. Responsible for oversight of Ryan White Part B and C Grants. Coordiantes pre-award budget and reporting. Manages post award budgeting and reporting. Ensures timely reporting of quality initiatives and maintains polices and procedures according to Grant regs.
Minimum Qualifications Required
Education: Bachelor's Degree in Human Services field, Bachelor's degree in non Human Services and 2 years case management experience, 2 year associate degree in Human Services field and 4 years case management experience.
