The Behavioral Health Specialist must be a Licensed Clinical Social Worker (LCSW) and is the primary interface for patients to access services such as peer/family counseling and support, screening, assessment consultative services, education and engagement, linkages, outreach and advocacy. Working from an interdisciplinary approach, the LCSW works closely with clinic providers to facilitate behavior health services to patients.
Essential Functions and Basic Duties:
Provide licensed level behavioral health services in low-intensity / brief treatment, problem-solving therapy, and behavioral activation in a culturally and linguistically and age appropriate manner.
Develop, implement, and evaluate plans for the provision of mental health and support services jointly with the Medical Provider(s) and other caregivers as appropriate.
Provide early interventions and referrals to specialty mental health service, including referral for more intensive psychological or psychiatric support services, if needed.
Provide training for providers and staff on behavioral health topics as needed.
Clearly communicate to other health personnel either orally or in writing the findings and planned treatment.
Able to collaboratively communicate with referring physicians.
Positive and supportive team member to staff members, clinic and hospital.
Strive to ensure continual compliance with Departmental and Organization Policies and Procedures and State/Federal Regulations.
Records, compiles, and prepares reports and summaries
Can identify & access system to report abuse to employee and/or patients.
Attend required medical in-service training as needed.
Performs Other duties as assigned.

CAC I, II, or III preferred, or must be willing to obtain with employer assistance
