Under the direction of the Ortho/Spine Service Line Business Manager, clinical manager and the Medical Directors of the Total Joint and Spine Center, the Ortho/Spine Nurse Navigator will assist surgical patients and their caregivers on their journey through orthopaedic/spine care as an advocate, educator, coordinator and clinical liaison. The navigator will serve as primary point of contact and is responsible for coordinating the care and services of the identified patient populations for an extended period of time across the continuum of care. He/she will promote effective resource utilization and assume a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.
Graduate from a NLN or AACN Accredited nursing program.
Bachelors of Science in nursing preferred (or currently enrolled in program)
WORK EXPERIENCE REQUIREMENTS
Minimum of 2-3 years of experience in ortho/spine nursing in the inpatient and/or outpatient setting
Experience/knowledge in community health setting/services desirable
Current RN license to practice in the state of Colorado
BLS for healthcare providers certification
Orthopedic nurse certification (ONC) preferred (or obtain within 1 year of employment)
Light Work - exert/lift up to 20 lbs. force occasionally, and/or up to 10 lbs. frequently.
