ThePANN is the key link between SMH and referring physicians and hospitals. S/Heobtains clinical information and physician orders from referral sources,determines appropriate level of care, assigns patient status (admission,observation or outpatient), and coordinates the appropriate placements ofpatients requiring admission, transfer, or for clinically complex outpatientservices. The PANN is also responsible for working with Dean Health Planpatients to redirect them to appropriate in network resources. Safe andappropriate placement of patients maximizes throughput and capacity andenhances fiscal accountability.
- The PANN acts as a resource and liaison between the Emergency Room, Nursing Units, Physicians' Offices, Urgent Care, Outlying Hospitals, Administrative Supervisor, and other sources of patient admissions and inpatient units. The PAN will facilitate and coordinate patient access, patient placement and communicate with the Administrative Supervisor assuring quality patient care and a safe environment for patients, families and employees is achieved. Communicates with Dean Health Plan to facilitate transfer of members to in network facilities.
- Track and monitor pending discharges and observation status as part of the coordinated patient placement strategy incorporating bed management and InterQual level of care technology.
- Ensures compliance with hospital standards, policies and procedures, as well as accepted professional standards and regulatory guidelines.
- Maintain and foster professional relationships within both the hospital and the community. Support communication and interaction with Internal and External partners including physicians and nurses to facilitate patient access.
- Provide feedback to staff and managers regarding staff performance.
- Supports stewardship through fiscal responsibility, monitoring patient activity and level of care and related staffing resource allocation. Continually evaluate operational systems of care and offer feedback for improvements to appropriate individuals, managers, or groups as needed.
- Coordinate patient placement for appropriate level of care. Monitor utilization of appropriate resources necessary for delivery of quality care. Identify throughput issues and develop workflows to improve processes.
- Assures timely response to physician/outlying hospital referral requests and links MD caller to accepting physician.
- Support department initiatives to improve quality, enhance processes and reduce costs where possible. Participates in process improvement activities within the department by using CQI tools and principles.
- Obtains pertinent information from referral source, communicates with referring and receiving care team members to assess patient needs for appropriate placement and facilitates timely admission to the appropriate nursing unit.
- Communicates with Administrative Supervisor to assure necessary resources are made available.
- Licensure (as required below) signifies education requirements have been met.
- Current licensure as a Registered Nurse in the State of Wisconsin.
- 3-5 years acute clinical nursing experience required
- Experience with eMR required
- Bachelors' Degree is required
· Case Management Experience preferred
