Hours: This position has on-call responsibility with an occasional holiday on-call support. This is a part-time position covering shifts from 8:00am - 4:30PM or 9:00 am-9:30pm. Occasional weekends as needed.

Primary Responsibilities: As a member of the Interdisciplinary Team, the Social Worker assists patients and their families to identify individual outcomes, manage and gain access to necessary health prevention, medical, rehabilitation, social, health education, financial resources, and community services. Social Worker care management includes initial and ongoing assessment, care plan development, ongoing monitoring, risk reduction strategies and education and service coordination. Emergency Department assignment with support to PHC Social Work off hour's needs. Other responsibilities include assisting with multisystem advocacy to navigate legal, health and government systems.
- Masters of Social Worker required.
- Current State Licensure required.
- LCSW preferred
The ideal candidate possesses:
- 1-2 years hospital experience preferred
- Excellent interpersonal communication and negotiation skills.
- Strong analytical and PC skills.
- Experience with pre-acute and post-acute care as well as community resources.
- Ability to work independently and be collaborative.
- Promotes safety cost efficiency and a commitment to quality improvement.
- EPIC proficiency desirable.
