1. To seek and find isolated, frail Will County residents, aged 60+ to inform them of available opportunities and benefit programs for which they may be eligible, and which will help them maintain their quality of life.
2. To facilitate access to and linkage with those services, opportunities, and benefits by making home visits to those persons unable to come to the Senior Services Center of Will County or any of its satellite sites.
3. To clarify needs and access, reassess, and manage long term care clients for Community Care Program services or nursing home placement under the auspices of Illinois Department of Aging and Medicaid waiver.
Must possess a B.S.W. or Bachelor's degree in human service field. Bilingual/Spanish speaking is preferred.
