The CHICO Clinical Social Worker will be responsible for:
Working directly with PPOC practices in the region through the Care Coordinators in each pediatric primary care setting to enhance and build capabilities of each practice to better educate and direct care to those areas of most identified patient need.
Supporting care coordination effort around successful deployment of high risk screening tools with the goal of identifying patients/families at risk for poor outcomes and barriers to care.
Developing and providing trainings and educational sessions targeting the needs of each practice and their patient population.
Developing strong working relationships with each practice and community based programming supports to enhance connectivity and patient care.
Identified target area: North of Boston including, but not limited to, Lowell, Malden, Medford, Revere, Winthrop, Peabody, North Andover and Lexington.
To qualify, you must have:
A Master's degree in Social Work and a license in the state of MA.
An LICSW and a minimum of 2 years post LICSW Social Work experience, including knowledge of hospital systems and hospital-patient care.
Fluency in Spanish, highly preferred.
The ability to travel among primary care locations as needed.
