The WWAMI Practice Transformation Network (WWAMI PTN) is a program awarded to UW Medicine by the Center for Medicare and Medicaid Innovation under the Transforming Clinical Practices Initiative (http://www.healthcarecommunities.org/CommunityNews/TCPI.aspx). The program is designed to create collaborative peer-based learning networks to engage clinicians in leading and creating positive change for the entire health care system. The program will assist clinicians in changing the way they deliver care by integrating quality and process improvement to build on and spread existing change methodologies, practice transformation tools, published literature, key learnings, and technical assistance programs. The program is one of the largest federal investments designed to support clinician's in healthcare delivery transformation, and presents a unique opportunity to significantly engage practice and providers across the WWAMI Region to achieve a mutual goal of patient-focused care.
The UW Medicine Office of the Associate Chief Medical Officer has an outstanding opportunity for a full-time, temporary Practice Consultant.
In conjunction with the UW Medicine Office of the Associate Chief Medical Officer, the Practice Consultant supports organization participation and practice transformation in the WWAMI PTN award. Specific areas of responsibility are in two major categories:
Coaching and Consulting on Practice Transformation
Organization Participation Management
This position is part of a new work in rapid development and deployment. The work assignments for this position mean that the incumbent will be part of a dynamic work environment and experience a particularly high degree of change and ambiguity. A successful candidate must embrace this environment and be able to function at a high level in this context.
Coaching and Consulting on Practice Transformation
Help organizations to plan for change by guiding the development of a Care Transformation Plan abiding by TCPI guidelines to assist practices in moving through the phases of transformation. Provide coaching and consultation on effective performance improvement strategies:
Support enrolled organizations in the development of a practice improvement plan to help move them through the phases of transformation included in the practice assessment; accomplish this by utilizing organization's quality improvement plans, practice assessments and other information gathered to assist with the development of the practice improvement plan
Link PTN offerings to clinicians and practices in a cohesive and well-planned manner
Work closely with organization's leadership to strategize implementation and spread of improvement initiatives
Consult with organization(s) on barriers and challenges encountered in making changes; trouble-shoot solutions and assist in creating and articulating resolution plans.
Provide project management support to ensure action items are completed and support and resources requested of the PTN are provided to the enrolled organization.
Organization Participation Management
Act as a resource liaison with participating organizations:
Develop a broad and deep understanding of all PTN training, tools and resources and TCPI national curriculum; match with organization's care transformation needs
Assist with developing and managing a resource matrix of TCPI offerings
Research and procure supporting information and documentation about training, tools and resources
Produce sub-reports of available training, tools and resources upon request
Assist with practice assessments of enrolled PTN organizations in coordination with practice leadership and local Quality Improvement Organizations. Participate in assessment meetings to track areas of focus for the enrolled organization and to document assessment results to be submitted to TCPI. Assist with creating assessment summary reports to be shared with enrolled organizations.
Accurately track organization participation in PTN training, tools and resources and/or TCPI national curriculum and change package; capturing, highlighting and reporting out on practice activities that are successful in achieving practice transformation goals.
Coordinate processes and procedures for external practice participation in WWAMI PTN, including but not limited to enrollment rosters, practice assessments and practice payments.
Serve as a liaison to the Transforming Clinical Practices Innovative collaborative, including but not limited to attending teleconferences and webinars related to the award and serving to share out relevant information about WWAMI PTN, as well as actively intake and incorporate emerging award information to inform WWAMI PTN operations.
Assist with managing WWAMI PTN healthcare communities portal presence, including weekly scanning and downloading of emerging content and information to inform WWAMI PTN operations, posting submissions to the WWAMI PTN sub-page, and managing subscribers.
Coordinate exploration with external collaborators to assess opportunities for synergy with WWAMI PTN and to avoid duplication of efforts, including research and compilation of information and development of collaboration plans as appropriate
| Bachelor's Degree in social sciences, health sciences, public health, health administration, or related field. |
2 - 3 years of healthcare system experience with progressive responsibility
2 3 years of experience in project management, quality improvement, process improvement or related organizational management responsibilities (preferably in a healthcare setting
Demonstrated experience and skills in building positive and productive relationships
Can see large picture, but zero in on details
Resilient, agile and able to work in dynamic environment and under pressure
Excellent organizational skills
Ability to manage/juggle programs and projects at multiple stages, keeping the overall goals in site without important milestones and timelines dropping from view
Excellent problem-solving and critical thinking skills, strong detail orientation, and ability to act independently and adeptly manage multiple priorities and timelines without close supervision
Excellent verbal, written, and interpersonal communication skills; Able to independently determine appropriate and effective communications and procedures
High degree of comfort with ambiguity and change, and ability to remain flexible, problem solve, and exercise judgment in a highly dynamic environment

3 5 years of experience in project management, quality improvement, process improvement or related organizational management responsibilities (preferably in a healthcare setting)
Experience as an external consultant
Experience serving as a practice consultant helping to develop care transformation strategies and assist with implementation of such strategies
Knowledge and experience with the development and implementation of Accountable Care Organization models
Knowledge and experience with the development and implementation of Patient Centered Medical Home models
Experience with researching or analyzing the landscape for, and then maintaining and updating, a dataset of tools and resources, and directing queries to the resources appropriate to their variable needs
