The Sinai Urban Health Institute (SUHI) is hiring a bilingual(English/Spanish) Community Health Worker (CHW) with previous health educationexperience to work across SUHI's health interventions related to breast health,asthma and diabetes. Under the direction of the Supervisor of Program Initiatives, the CHW willbe responsible for providing and coordinating health education, as well as casemanagement services. The CHW will serve as a bridge between the patient and themedical system by building trusting relationships with community members servedby the program.
Specifically, the CHW will beresponsible for community outreach and providing health education in a varietyof settings including health fairs, community events, group sessions, and otherformats as needed. The CHW will also help in identifying barriers to accessingquality care, work with individuals to overcome these barriers, and providereferrals. For asthma, the CHW will work closely with program participants intheir homes to provide comprehensive asthma and healthy homes education. In thearea of breast health, the CHW will work with women to obtain timely androutine breast health screenings, and assist women with navigating the healthcare system to obtain screening results and appropriate diagnostic andtreatment services as needed. Fordiabetes, the CHW will help clients create personalized wellness plans andpromote diabetes self-management. Activities will also include programpromotion, enrollment and collecting health information from those interestedin SUHI's programs.
SUHI is seeking an enthusiastic,passionate CHW candidate that has previous experience in community healtheducation and meets other job requirements outlined below.
- Must have high school diploma or GED
- 2 years health education experience preferred
- Familiarity with and experience working in Chicago's Westside communities (resident of Westside communities preferred)
- Passion for and dedication to working with community members
- Punctual, reliable and willing to learn
- Must have a valid driver's license and own a car
- Proficiency in Microsoft Word, Excel, Access and PowerPoint preferred
