Community Health Partnership of Illinois (CHP), a non-profit organization that provides primary health care to migrant farmworkers and other medically underserved populations throughout northern and central Illinois, is seeking a dedicated individual who is looking for an opportunity to make a difference in the health of a community in need.
Develops, manages and conducts health focused outreach to agricultural workers and their families in the Mendota service area
Recruits and supports community health workers to provide health education and advocacy
Coordinates with growers / employers of agricultural workers (external) and clinical staff (internal) to schedule Outreach Medical/Dental clinics and events.
Bilingual; Spanish/English and ability to relate to the target community
Strong communication & organizational skills, a minimum of HS Diploma, advanced education a plus.
Ability to work independently and as part of a team.
Valid Driver’s license and automobile insurance.
Additional Salary Information: variable hours due to seasonality of position
