CUHCC’s mission is to seek health equity in our community by advancing the well-being of diverse people. This position directs the continuum of care coordination services provided at CUHCC. The position manages CUHCC’s Behavioral Health Home services, ensuring that CUHCC retains its certification and that all BHH services are billed to maximum capacity. This position will also work closely on objectives outlined by the SAMHSA grant. This position also directs the care coordinators to ensure that CUHCC meets its FUHN targets of reducing total cost of care. This position manages the Office of Justice grant, which include supervising the victim advocates. This position may provide some direct care to patients in BHH, SAMHSA, and NAVIGATE program.
This position reports to the Behavioral Health Director and directly supervises care coordinators (which include social work interns) and DV/SA advocates.
ESSENTIAL FUNCTIONS & PERCENTAGE FOR EACH FUNCTION:
1. PROGRAM DEVELOPMENT AND IMPLEMENTATION (30%)
-Design CUHCC’s Behavioral Health Home Care Coordination Program to provide care coordination and mental health education for CUHCC’s patients with SPMI/SMI who are not enrolled or not eligible for Case Management or ARHMS services.
-Work to ensure that care coordinators improve patient, clinical and financial outcomes.
-Work with other CUHCC stakeholders to design and implement a set of metrics for BHH and SAMHSA with annual goals.
-Develop an annual workplan for Care Coordination.
-Determine optimal/practical staffing and qualifications for care coordination.
-Create job descriptions and staff competency expectations.
-Write and implement detailed standard work flows for care coordination.
-Design a training program for care coordinators to meet care coordination needs.
-Analyze and recommend care coordination, education and other software and technology. Implement software as possible.
-Determine workloads for care coordinators. Incorporate these expectations into the work flows, hiring expectations and staff evaluations.
-Determine annual workplan for DV/SA advocates.
-Develop the process to access and select social work interns and the social work triage position. Interview and select interns.
-Work with the liaison from the School of Social Work to oversee and monitor the work of the social work interns including the completion of necessary paperwork and onboarding.
2. SUPERVISION AND MANAGEMENT (30%)
-Hire, assign, and train care coordinators to achieve the goals outlined in the annual care coordination workplan.
-Determine and assign patient panels.
-Disseminate BHH tasks to care coordinators and ensure that metrics are being met.
-Participate in FUHN CQI/Care Coordination meetings.
-Working directly with care coordinators and indirectly with other staff, ensure that CUHCC is meeting BHH requirements.
-Ensure that CUHCC is optimally billing for BHH work.
-Ensure that a maximum amount of patients receive care coordination services.
-Monitor care coordination metrics on a daily/weekly/monthly basis.
-Define performance standards for direct reports that represent individual and team contributions to business outcomes, cultural competency, and patient and staff satisfaction.
-Ensure that care coordinators receive continued training and skill development.
-Hold regular meetings (individual and team) with direct reports to ensure staff engagement, performance, and satisfaction. Ensure that meeting minutes are taken and filed.
-Review the performance of care coordinators on an ongoing basis. Record conversations, kudos, coaching and other pertinent data into the care coordinator’s supervisory file.
-Work to develop a high performing care coordination team that is responsive to patient needs.
-Provide supervision to Social Work interns and Social Work Triage position.
-Participate in Quality, CLOPs and other meetings.
-Develop a monthly dashboard report for care coordination, DV/SA, and social work.
3. DIRECT CARE (30%)
-Provide direct care, including therapy and diagnostic assessments, to participants in BHH, SAMHSA, and NAVIGATE programs.
