Provide case management services to grant participants, ages 16-24 in the Kendall County and Aurora area, in order for the participants to gain access to educational and employment opportunities which may not otherwise have been available to them.
Essential Duties and Responsibilities
Recruit, interview, and determine the eligibility of potential youth participants;
Performs assessment and initial intake of program participants, following up as necessary to obtain all appropriate documentation;
Administer a career assessment with each participant in order to assist them in identifying their areas of interest;
Assess the training and education needs of individual participants and design and implement their career development plans;
Develop and modify an Individual Service Strategy (ISS) with each participant, including assessing each participant’s need for appropriate supportive services;
Conduct professional ongoing case management/career/life counseling for participants from program recruitment and enrollment through their exit and 12 months of follow-up services;
Work closely with the Youth Services Team on the planning and implementation of cohort industry specific programming that includes Job Readiness boot camps and internships off site;
Evaluate participants’ on-the-job performance either through observation or with the work site employer
Research and identify the best training and education program for individual participants;
Researches and coordinates use of outside resources to benefit individual participants.
Maintains records in keeping with regulatory requirements.
Provides reports as requested.
Performs other duties and takes on other responsibilities as requested.
Qualifications and Education Requirements
Degree or equivalent experience in education, career counseling, or a closely-related area.
Ability to gather, organize, and analyze information.
Excellent communication and counseling skills.
Ability to work with large amounts of paperwork, as required by funding sources.
Overall knowledge of grant regulations and the importance of compliance with them, or ability to learn quickly.
Team orientation with respect for others, as well as ability to work independently when necessary.
Ability to work well with individuals of various social and economic backgrounds.
Travel into Kendall County and Aurora area is required.
Extensive training will be provided in the main office in Arlington Heights. Must be willing to travel to Arlington Heights.
