Social Worker positions at CHANS Home Health and Hospice are medical social services positions. Social Workers collaborate with registered nurses and other members of the Hospice interdisciplinary team to enhance understanding of the significant social and emotional factors related to the patient’s health problems, and to identify and facilitate solutions.
Hospice Social Workers primarily work with Hospice patients, while Social Workers primarily work with Skilled Services patients. Since Social Workers and Hospice Social Workers cover for each other’s roles, both positions are held to the same Job Description requirements.
This position will have the following responsibilities:
Assists patients and their families to adapt and plan in the home environment
Relief of stress
Advocacy with community agencies
Assistance with financial problems
Information and referral
Assistance with planning, emotional support, appropriate counseling
Interpretation of and education about patient and family needs with other health care team members
Develops and maintains a professional yet personable relationship with clients and families which enhances the identification and fulfillment of the client’s needs.
Bachelor's Degree in Social Work or related degree, required. Masters Degree for an accredited college preferred.
Maine State LCSW
One year minimum social work experience in related area
Community health experience preferred
Completion of a four-hour orientation specific to hospice service
