We are currently seeking a dedicated palliative Social Worker to work within a specialized interdisciplinary team in inpatient and outpatient settings to assess patients and families and to provide support and consultation related to their diagnosis, illness and psychological needs.
You will need:
Education: Master's degree from an accredited social work program required
Current, valid New York State Licensed Master Social Worker (LMSW) required
Licensed Clinical Social Worker (LCSW) or R-License and ACHP-SW preferred
Demonstrated evidence of experience in palliative care setting, or related work such as oncology and/or hospice. Specialty training or 3-5 years of inpatient, outpatient or home-based social work required. Palliative social work fellowship or post masters palliative certification desired.
Solid working knowledge of social work practice, relevant medical and/or legal issues that impact patient care, and medical center resources to access and provide for patient care needs; Excellent communication, interpersonal, assessment, counseling, multitasking, detail orientation, and organization skills; Basic computer skills including Microsoft Office especially Word, Excel and PowerPoint required.
