The Substance Use Counselor is responsible for an assigned caseload, or sharing a caseload through participation on a treatment team, in order to facilitate the development and implementation of individualized treatment plans for persons with substance use disorders. The Substance Use Counselor provides substance abuse assessment, treatment planning and service delivery grounded in evidence-based practices to assist individual with substance use disorders to achieve rehabilitative and recovery goals.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Takes a clinical role in making sure that substance abuse services are provided to members. Competency in the following treatment modalities is essential: individual and group counseling/therapy specifically designed to treat substance use disorders, case management, crisis intervention, family psychoeducation, psycho-social rehabilitation services on site at a Thresholds location, and treatment of co-occurring mental disorders when indicated.
- Makes sure substance abuse admission and discharge assessments, treatment plans, continued stay reviews, and progress notes are completed in a timely manner, in accordance with Thresholds policy and DASA standards.
- Participates in team discussions that result in developing an integrated treatment plan, based on the assessment data and the goals the member has identified as most important to him or her. Ensures the treatment plan addresses the member’s stage of treatment, clinical needs, and includes objectives and techniques for accomplishing each goal. Also, treatment plan development may require the Substance Abuse Counselor to promote cooperation with the psychiatrist, other team members, the member, their families, and collaborating agencies.
- Provides stage-wise approaches to treatment, including evidence-based strategies for pre-contemplation (harm reduction), contemplation (motivational interventions), preparation and active treatment (planning for sobriety and skills for sobriety) and relapse prevention planning and implementation, including identifying risk factors related to the relapse and development of relapse prevention plans and strategies.
- Where members have co-occurring mental disorders, helps make sure that integrated services remain accessible to all members, regardless of their stage of treatment, or “readiness” to engage in services.
- Encourages consumers in later stages of treatment to attend mutual aid groups, including AA, NA, SMART Recovery, CA DRA, Double Trouble, and DDA. Uses 12-Step Facilitation practice and promotes shopping for a mutual aid support group that meets the preferences of the consumer and helps process their experience afterward.
- Works collaboratively with the Quality Department to assure that documentation of assessments, goals, treatment plan, and interventions is in accordance with Thresholds practices.
- Intervenes with collateral sources, agencies, bureaucracies, facilities, and families on the members’ behalf. Advocates and assists with problem solving/resolution that will help the client access and utilize support from the community or support system, including family.
- Develops resources within the agency and/or community.
- Performs and participates in crisis intervention when necessary.
- Works cooperatively with inpatient hospital and other collateral staff, keeping logs and other records of all contacts as required by agency practice
- Monitors self-administration of medication, when indicated by the DMH license for people with co-occurring substance use and psychiatric disorders.
- Follows all Thresholds procedures regarding the handling of member’s funds. All safeguards, reports and accounting of custodial funds, petty cash, and member maintenance funds meets the highest standards and follow the policies of Thresholds.
- Meets required billing targets and completes all documentation within assigned time frames.

MARGINAL DUTIES & RESPONSIBILITIES:
- May be required to oversee and maintain the safety of the members, staff and visitors. This requires the upkeep of fire and safety equipment and document accordingly. Conduct all safety drills in conformance with current CARF and Agency standards and complete documentation as instructed.
- May be required to communicate significant information regarding members to other staff workers and supervisors. Provides accurate and timely information to your supervisor on all topics and issues related to the operation of the agency and the well-being of members.
- May be required to work flexible schedules and be available to work on weekends or evenings as required.
