Title: Southside Drop-in Case Manager
Reports To: Program Coordinator

We provide access to stable housing and deliver comprehensive services that act as a catalyst to transform lives and communities.” La Casa Norte’s mission and philosophy of service is to impact the social issues of homelessness at the individual and community levels.
Casa Corazón is aimed at transforming the lives of homeless and at-risk youth, ages 16-24, by providing them with the stability, support, and guidance they need in order to become thriving members of society. Program participants will receive street outreach and access to a variety of services including a drop in center with youth programming and meals, basic needs such as transportation and toiletries, and case management services focusing on a holistic, strength-based approach, with a focus on housing stability and educational, job readiness and placement.
This opening is for the Southside office of Casa Corazon located in the Back of the Yards community. This is a full time position.
Expected Goals and Objectives
Goal One: To provide comprehensive advocacy and drop-in services to homeless youth ages 16-24
- Maintain professional relationships with all staff and clients
- Support coverage and coordinate activities as necessary during drop in hours.
- Conduct outreach on the west side to: schools, churches, other community service providers and locations that homeless youth congregate both formally and informally.
- Assist Program Manager in the management of appropriate program development and effective service delivery.
- In partnership with participants to assess all barriers to safe and stable housing and provide referrals and access.
- Providing life skills training, counseling, support, and guidance to Youth as they work to overcome barriers to housing, employment and education.
- Take advantage of key “teachable moments” to help youth develop skills in decision-making, conflict resolution, time management, etc.
- Provide effective case management services to program participants utilizing a “strength-based” case management model and monitoring participants progress through modeling, mentoring and positive relationship building
- Referring Youth to other supportive services as needed, including food pantries, clothing closets, legal aid services, parenting classes, substance abuse and mental health counseling, etc; coordinating these services as needed to obtain wrap-around supportive services for clients.
- Advocating for clients as needed to shelters, landlords, employers, and other agencies or institutions.
- Provide crisis intervention services to Casa Corazon youth as necessary
- Accurately tracking and documenting all services provided utilizing LCN Tracking system and Client Track.
- Plan a monthly schedule for events, activities, and food menu.
Goal Two: To work in a team approach to maintain a positive and safe milieu at Casa Corazon
- Observes environment to ensure safety of youth and staff by responding to emergencies, building repairs, safety hazards, crisis and incidents.
- Maintains a safe and healthy environment for staff and Youth through shared responsibilities of cleaning. Encourage Youth to share in cleanliness of space through shared responsibility.
- Maintain on-going dialogue with staff around areas of youth successes and challenges.
- Follow up with any incident in a timely manner utilizing a strength base approach.
Goal Three: Case Management Duties
- Provide intake and assessment services.
- Assist with applying to public aid benefits and obtain any necessary documentation.
- Provide short term case management, including budgeting assistance, service planning, advocacy and referrals.
- Use discretion to distribute CTA pass to youth if transportation is needed for employment, housing, appointments, or chore completion.
- Identify housing opportunities for youth, (shelter, apartment, supportive housing program etc).
- Document all services in HMIS, Client Track, and paper file.
- Collaborate with other internal programs to provide assistance to youth.
Goal Four: Administrative Responsibilities
- Provide the overall leadership and planning to operate drop-in services.
- Coordinate staffing, workshops, food and programming.
- Supervise all Drop-in staff, interns and volunteers.
- Will maintain organization and inventory of all toiletries, food, bus cards and other essential items.
- Teach and implement the declaration of non-violence at Casa Corazon.
- Provide clinical supervision to MSW and other graduate student interns as appropriate
- Intermittently gather and complete data entry for various grant reports and development purposes.
- Work in conjunction with Program Supervisor to meet program outcomes.
In addition to the responsibilities listed above, all La Casa Norte employees are responsible for contributing to the overall success of La Casa Norte by:
- Attending weekly staff meetings and other staff trainings.
- Participating in La Casa Norte’s fundraising events.
- Participating in on- and off-site staff trainings, workshops and outreach events as needed.
- Perform other duties as needed.
