Job Posting: Cedars-Sinai Medical Network (CSMN) is committed to delivering effective and efficient quality health care to our patients with chronic pain who are being managed in the primary care setting. The CSMN does this through a collaborative care model that embeds a dedicated team of healthcare professionals with expertise in treating chronic pain within our Primary Care Medical Home. The Physician Assistant (PA) serves as a key member of the Pain Management Program (PMP) team working closely with: 1) Primary care physicians; 2) PMP Medical Director, 3) interdisciplinary members of the team e.g., clinical pharmacists, mental health specialists, care managers, program coordinator; and 4) referral specialty network providers including therapists, interventional pain specialists and surgical specialists across the Cedars-Sinai Health System. Some treatments will be delivered by the PMP team while other treatments will be delivered by referral to network providers in a multitude of locations. Given the high prevalence of psychiatric co-morbidities seen in chronic pain conditions, the staff of the PMP will interface closely with the staff of the primary care based collaborative care model Mental Health Program to integrate and coordinate care for those patients requiring services from each of the respective disciplines. The target pain population includes patients with a variety of chronic pain conditions including spine, neuromuscular, musculoskeletal, neuropathies, HA/migraine, abdominal pain, pelvic pain, or chronic pain resulting from complex medical or surgical treatments. The target mental health population includes patients with depression, anxiety, and other behavioral conditions commonly treated in Primary Care. Medication management will be a major component of both programs. ESSENTIAL JOB DUTIES:
Under the direction of the PMP Medical Director, conduct patient assessments, order or directly provide treatments, and provide patient education and support
Work under protocol to develop an effective, safe and patient-centered treatment plan
Provide timely and complete clinical documentation in the EHR following patient visits or telephone calls
Under the direction of the PMP Medical Director, assess patient self-administration of pain medications including performing CURES checks, urine drug test screening, symptom response, and functional improvement per standardized assessment tools; Working under protocol is able to titrate medication(s) as indicated
Maintain ongoing communication with the PMP Medical Director regarding assigned patients and provide timely notification of changes in patient status
Attend case conferences, clinical rounds, and other types of meetings which address patient management and care coordination
Maintain ongoing communication with other members of the care team through written documentation, telephone or in-person conversation.
Partner with the patient to develop contracts of care as needed and provide encouragement and motivational support to promote adherence to treatment regimens and behavioral plans
Upon adequate training, may be engaged in teaching patients to incorporate principles and techniques of cognitive behavior therapy (CBT) in their self-management of chronic pain through individual patient appointments as well as group classes. Demonstrates a holistic approach to treating chronic pain by considering a broad range of interventions as well as patient preferences E.g., Aware of full scope of musculoskeletal therapies, PT, hydrotherapy, acupuncture, mindfulness, meditation, CBT

Able to obtain C-S Medical Care Foundation Allied Health Professional privileges (as delineated within the "Privilege Delineation Description and Standards" document).
Minimum of 2 years of relevant work experience
Demonstrated commitment to the concept of working on an interdisciplinary team
Experience working with chronic pain patients in a primary care setting strongly desired
Experience in medication management of a chronic pain population including initial and ongoing risk screening and refill monitoring
Competent to assess a broad range of pain conditions and problems and to use experience, professional judgment and clinical algorithms or pathways under the supervision of the PMP Medical Director to guide care of the patient
FT position, Monday – Friday. Ability to attend occasional early morning or evening meetings. May be asked to cover off-hours patient calls on a rotating basis.
Workers Comp experience desirable
Knowledge of managed care principles and population health practices and tools desirable.
Computer competency comprised of a working knowledge of Windows and/or a comparable operating system that sufficiently provides for access to patient and other business information
