Erie Family Health Center is looking for a focused, strategic individual like you to join our Patient Care Team! The Medical Care Manager works independently and as part of a team to support the needs of Erie’s Patients.
Work with eligible Erie patients who are high and medium risk on their health and well-being
Assist medical providers, clinical staff, the behavioral health team, and other care managers to identify potential clients
Complete initial and on-going assessments, and create linkages with medical, behavioral, and supportive services.
Provide timely and valid information to the medical team related to on-going and follow-up care
Assess patients at risk for depression and continuously reassessing their progress every two weeks: following the guidance of the behavioral health team and Collaborative Care Model Coordinator.
Document all behavioral health assessments and communicate progress as needed to ensure continued follow up
Assist in outreach, advocacy, assessments, care plan, and data entry
Bachelor’s Degree required in (social work, community health, or related field)
Bilingual (English/Spanish), understanding of medical conditions and preventive healthcare required, a minimum two years work experience in health, social, or community services preferred
A minimum two years of work experience in health, social or community services preferred
Strong computer skills: expert in Microsoft Office, Excel and ability to learn to expert level other electronic databases (Electronic medical record system, clinical connect)
Excellent written and oral communication skills: must be able to communicate promptly and professionally to several levels of various organizations and coordinate communication between individuals
