These individuals will:
Under general direction assign International Classification of Diseases
(ICD), Current Procedural Terminology (CPT) and Evaluation & Management
(E&M) codes to diagnoses and operative procedures for optimal physician and
facility fee billing. Perform data entry and abstract pertinent data from
medical records to insure data integrity and optimal reimbursement. Design
quality assurance studies and abstract data; perform data verification and
transfer data between computer systems.
1. Analyze clinical data and interpret information.
2. Assign ICD-9-CM codes to complex trauma diagnoses and procedures in
an integrated system of outpatient records.
3. Abstract patient data to insure data integrity, accurate
reimbursement, proper case mix and hospital decision support.
4. Review patient records such as nursing documentation and treatment
records, to determine the charges for that visit.
5. Complete a professional fee sheet with the proper E&M level codes
for each record reviewed utilizing physician fee coding guidelines to assure
proper billing of each record.
6. Investigate and research billing and coding issues as related to
inquiries, complaints, or audit results.
7. Serve as a resource for current coding, billing and regulatory
8. Perform audits to verify accuracy, integrity and quality of data.
9. Evaluate accuracy and completeness of supporting documentation.
10. Provide support to medical center physicians and staff for
documentation and coding review questions.
11. Identify the integrity of the documentation in the patient records.
Query physician for clarification of documentation issues and request record
addendum to make that clarification.
12. Assist in the development and maintenance of policies and procedures
13. Assist in the development and maintenance of the coding manual and
14. Communicate with providers who are clarifying documentation for
areas such as a problem list, reason for visit, care plan, diagnoses,
procedures and pharmaceuticals.
15. May direct or coordinate the work with others staff members.
16. May train new staff at times.
17. Perform special projects and related duties as required.
| Certified as a Registered Health Information Technician (RHIT) AND one year of ICD and CPT coding experience using a broad knowledge of coding standards and guidelines OR equivalent education/experience. |
| One year coding experience at a level I trauma center, to include burns, multiple trauma, Neuro/Ortho surgical catastrophic injury related diseases and/or medical complications using a broad knowledge of coding guidelines and standards. |
Proficiency with PowerPoint, MS Word and Excel.
-Experience or education in utilization management and InterQual criteria, a plus.
-Experience with health care billing, coding, and compliance, a plus.
-Excellent and effective oral and written communication skills.
-Self-motivated, detail-oriented and highly organized skill set.
-Equivalent work experience may substitute for education requirements.
-Comprehensive knowledge of Federal health care billing rules and regulations.
-Experience with E&M, CPT, ICD-9 and HCPCS coding.
