DUTIES:1.Provide supervision of patient navigators including training, evaluations, performance programs and time and attendance.2.Distribution of caseload and establishment of deadlines among patient navigators.3.Monitoring follow-up activities to ensure that monthly program targets are being met.4.Assist in the case review of patients for HQIP and High Risk patients as needed.5.Work with providers in the clinics to help improve NY QUARR/HEDIS scores.6.Help facilitate the use of patient registries, online managed care portals or other data tools.7.Ensure monthly submissions of supplemental lab data.8.Monitor specialty referral tracking and follow-up for labs, imaging and specialty referrals. Provide information to specialists or follow-up to obtain consults as needed.9.Conduct ongoing chart reviews to provide feedback to providers on HEDIS measures and improving outcomes on high risk patients.10.Identify and develop relationships with personnel in departments external to Ambulatory Care who are involved in patient's care to assist patient's navigating the system.11.Provide weekly reports on all quality improvement and HEDIS measure activities.12.Monitor participation in daily clinic huddles and follow-up on issues related to access to care.13.Assist with patient outreach and scheduling for special care coordination projects. (Collaborative Care at Family Medicine and Developmental Care for pediatric patients)14.Assist with all other patient navigators activities as needed.15.All other duties as assigned by supervisor.(DUTIES AND RESPONSIBILITIES ARE NOT LIMITED TO THE ABOVE POSITION DESCRIPTION)QUALIFICATIONS:REQUIRED: LMSW, Bachelor's Degree. 3-5 years working in a Healthcare setting. Proficient in Microsoft (Access, Excel, Powerpoint, Word).
