Responsible for addressing clinical, financial and social concerns for patients and their families with intellectual, developmental and behavioral health concerns. Provide leadership in the identification and acquisition of resources (local, state, and federal) for the support of families. Provide high level education and support for individuals and families for both clinical and financial needs. Serve as a resource person for patients and their families. Must be able to work effectively with diverse client groups and their families. Maintain statistics on completed work, patient notes and assessment’s using an electronic medical record including analysis of information for reporting purposes.
1. Interview and provide social work assessment, casework, education and counseling services to patients, families and caregivers. 2. Prepare a formal social history and treatment plan; coordinate the treatment plan with other members of the team. 2. Recommend social work services as appropriate to identified needs. 3. Follow up with patient or community resources to ensure access to or delivery of services. 5. Assists patient and family in connecting with resources for financial assistance, legal aid, housing, food assistance and health care coverage through private, city, state and federal programs. 6. Assists the patient and family to identify personal and environmental difficulties/safety issues and advises and educates on how to handle potential situations

Bachelor's degree in Social Work
2 years experience in Social Work or Human Services
Additional Qualifications Summary:
Knowledge of professional guidelines regarding confidentiality of client and staff information required. Competent in the use of a variety of computer applications including Microsoft Excel and Word. Ability to work effectively with diverse patient groups and their families required. Must demonstrate excellent communication skills both verbally and in writing required. Must demonstrate personal traits of high level commitment, motivation and energy, team orientation, and professionalism required. Knowledge of health care services and resources required. Ability to work independently required. Strong analytical skills required.
