The Community Hospice’s Palliative Care Program & SPHP Medical Associates, P.C. are teaming up to establish a new full-time Palliative Care Physician role targeted to be integrated with six of our primary care practices. The primary responsibility of this position will be to provide Palliative Care Consult Services to patients/families with serious illness referred by our primary care providers, including: providing a team-based approach to care with specially trained social workers and nurses to provide an extra layer of support focused on improving quality of life.
This position is a key component of the overall SPHP Palliative Care Service Line expansion plan aimed to provide palliative care shortly after diagnosis of a serious illness, to be available to patients and families over the course of the illness, and to integrate with our palliative care teams across settings (inpatient, home-based, outpatient, etc.)
