Advertised Summary Job Description: Provides clinical support and input to the Pediatric Infectious Disease and Rheumatology practices that is consistent with the goals of the institution and the department; develops a sustained environment of excellence in patient/family centered service.
Including but not limited to 1. Meeting with ID/Rheum team weekly to review lab results and plan and execute follow up plans 2. Coordinate care of ID and Rheum patients with email, crown notation and phone call follow up with other clinical services 3. Maintain outpatient follow up list (update and maintain on listserver) 4. Timely follow up with patients and families regarding test results and pending tests 5. Telephone triage of calls from patients/families in conjunction with ID/Rheum attending or fellow 6. Re-fill prescriptions IV and PO 7. Prepare orders in Crown (i.e. blood tests) for physician sign off 8. Coordinate care with Visiting Nurse Service and other outside agencies as needed 9. Coordinate referral to other specialties e.g. dermatology, immunology, etc. 10. Documentation of ID/Rheum plan in crown. 11. Coordinate with inpatient team to secure outpatient ID/Rheum clinic visit (meet with inpatient families when needed)
General Minimum Qualifications: Requires a bachelor's degree or equivalent in education and experience, plus four years of related experience.
Must be a NYS Licensed Registered Nurse.
Additional Specific Minimum Qualifications:
Preferred Qualifications: 1) Education: Baccalaureate degree in Nursing is required, Master's degree in Nursing or applicable health-related field is preferred 2) Competence: Preferably one to three years of clinical practice in the care of pediatric patients 3) Excellent interpersonal skills, both verbal and written, as well as creative problem solving capability 4) Certification in pediatrics is preferred
